Sign up: Click on “New User” if you are applying PTIN for the first time.

Form W-12: Fill out the online version of IRS Form W-12, PTIN Application.*
*Note* You only need to apply for a PTIN for the upcoming tax year. You will still be able to prepare previous year's returns with that same PTIN! -
Questions: Answer all questions regarding your tax compliance and felony convictions, if any.
